We welcome referrals from primary care physicians, hospitalists, and specialists throughout Madera County and the Central Valley. Fax is the fastest way to send us a referral — our staff will contact your patient to schedule once it's received.

Please include with your referral

  • Patient demographics and contact information
  • Insurance information (front and back of card)
  • Reason for referral and relevant clinical notes
  • Recent labs: creatinine, eGFR, CMP/BMP, urinalysis, urine protein if available
  • Recent renal imaging reports (ultrasound/CT), if available
  • Current medication list

Common reasons for referral

  • Elevated creatinine or declining eGFR (CKD stages 1–5)
  • Proteinuria or hematuria
  • Resistant or secondary hypertension
  • Electrolyte disorders (hyperkalemia, hyponatremia)
  • Diabetic kidney disease
  • Dialysis planning and vascular access coordination

For hospitalized patients, our physicians provide inpatient nephrology consultations at Saint Agnes Medical Center, Community Regional Medical Center, and Clovis Community Medical Center.
